MnCHOICES: Assessment for Services

MnCHOICES is a comprehensive, person-centered assessment that helps people with long-term needs determine support and service options available to them. The assessment is confidential and is offered to all interested individuals, free of charge and without regard to income. The assessment is typically completed in the individual's home with a certified assessor. It covers questions regarding health, safety, ability to complete daily activities, specific needs, strengths, and desires. Once the assessment is complete, the assessor will review eligibility for programs that may pay for services. The assessor will also be able to assist with making referrals to agencies that might also be of assistance.
