REMINDER - this must be completed on the appropriate website below:
You will want to have these documents available when you begin your reporting according to the MAT Attorney:
- Signed award terms and conditions agreement "Township resolution accepting the money" (some townships may not have resolutions)
- Signed Assurances document – download here
- Actual Budget documents – Levy document (levy set for 2020 that they submit to auditor's office) whatever they referenced as their budget/levy amount they
must use this number consistently throughout the process. (If you cannot locate please contact our office for a copy).
